Payment Options

FEES, INSURANCE AND FINANCIAL AID

OUTPATIENT FEES:

• Assessment -  $170.00
• Individual and family therapy – $150.00 per hour
• Group therapy and classes – $75.00 per hour
• Urinalysis $35.00 per sample

Note:   Scholarships and sliding scale available.

INPATIENT FEES:

Private Rate: $260.00 per day. Includes all treatment and consultation services delivered on site. Does not include medical or psychiatric services delivered by professionals in the community, such as at medical or urgent care centers.

Insurance Rate: Rates vary according to insurance carrier and contracts.

Insurance Coverage:

Daybreak services are covered by most carriers, including, but not limited to:

Premera Blue Cross, and Blue Card affiliates (Pre-authorizatin required)

Group Health Cooperative (Pre-authorization required)

Cigna Behavioral Health (Prior authorization required)

Aetna

United Healthcare

PHCO – Spokane (Referral to inpatient from an outpatient facility required; Pre-authorization required)

Daybreak services are not covered by Champus or Tri-Care.

Financial Aid:

Financial aid may be available to Washington and Idaho Residents who meet state or federal low income guidelines, or to families with serious financial hardships. The following programs are available:

Outpatient Services: Medicaid/Title XIX/CHIP coupons are accepted as full payment for services in Spokane.

Inpatient Services:

Medicaid/Title XIX/CHIP coupons are accepted as full payment for services – In Vancouver only.

State of Washington – Grants (Eligibility is determined by Daybreak according to DBHR guidelines.)

State of Idaho – Grants (Eligibility determined by Business Psychology Associates.)

Daybreak Scholarship Fund (These funds provided by United Way and private donors. Eligibility determined by Daybreak).
